The 1911 5¢ Washington (#378) was introduced in January 1911 with a perf 12 gauge. In December 1913 the new 10 perforation machines were employed at the Bureau. At some time, presumably 1914, the stamp were fed horizontally through the 12 gauge perforator and then, probably in error, fed vertically through the 10 gauge perforator. No date for this occurrence can be given as none of the surviving examples have a visible date on the postmark, There are two stamps with a Tampa Florida pre-cancel.
